How does this work?

How does the voIP service that allows me connect (deviously of course) two different land-line phones together work? Gizmo calls this Direct Call and I'm screwing around with it and was wondering how it works exactly.
Do you speak two languages?

I have some ATAs that have a direct connect feature. You just have to enter the phone menu and specify the IP of the ATA you want to call.
