How do you establish a voice bridge?

The title says it all. Did you have to purchase a phone number or do you use some type of service?

I've always just picked existing public conference numbers and told everyone to dial extension 752.  The people who own these numbers get paid for the incoming phone traffic, so that's why they exist.  I've never paid for a bridge number.
